Position Summary

The Living Well Coordinator (LWC) is the process owner of the American House Living Well Memory Care Program. This role is responsible for developing, implementing, and evaluating a person-centered activities program that promotes physical, cognitive, social, spiritual, creative, sensory, and life skills wellness. The LWC builds meaningful relationships with residents, families, and team members while creating a vibrant and engaging Memory Care environment.

Qualifications & Experience

  • Related degree or experience in Memory Care and/or Dementia Care.
  • Minimum of 2 years of dementia and memory care experience preferred.
  • Experience implementing programs for a Memory Care population.
  • Ability to lead activities for residents with varying cognitive and functional abilities.
  • Strong computer, organizational, and time-management skills.
  • Excellent communication, interpersonal, problem-solving, and teamwork skills.
  • Flexible schedule and a passion for supporting residents, families, and team members.
  • Experience motivating others through fun, engaging, and meaningful activities.

Primary Responsibilities

  • Develop, implement, and oversee a diverse, therapeutic 7-day-a-week activity calendar.
  • Lead all aspects of the Living Well Program and continuously enhance programming using the Memory Care Playbook and performance goals.
  • Coach and support team members on the importance of resident engagement and how to lead activities when the LWC is unavailable.
  • Conduct orientation for new team members on Living Well policies, procedures, and programming expectations.
  • Provide ongoing education and support for Memory Care team members and resident families.
  • Welcome and orient new residents and their families to the community.
  • Foster a culture of compassion, engagement, and person-centered care within the Memory Care neighborhood.
  • Perform other duties as assigned consistent with the position.
